  Monthly Religious Services & Programs Report (RSP) Form  
     
  Please read instructions below to better assist you in using this on-line form!  
     
 
 
 
     
 
The RFP Form is due by the 10th of every month (i.e. it should be submitted electronically between the 1st and 10th of the month). If you are mailing the hard copy version of the form, you will need to get it in the mail early enough to be received by the 10th.
The button on the LEFT takes you to the electronic version of the form. NOTE: You can only access this version if you’re working on a DOC computer (i.e. at a DOC facility). We urge you to try to use and submit this electronic version, as it will make the process quicker and easier.
The button on the RIGHT takes you to the printable hard copy (PDF) version of the form. Clicking on this button allows you to print out the form and fill it out by hand or to type on it. You can then either fax it to us at Chaplain Service at fax number (804) 358-7020 or mail it to us at: Chaplain Service of the Churches of Virginia, Inc.; 2317 Westwood Avenue, 103A; Richmond, VA 23230. Note that you can also use the printable version as a worksheet to do by hand (at home or to have a volunteer or an inmate clerk fill it out, etc.), and then you can use it as a guide to fill out the electronic
IMPORTANT: Offenders cannot have access to the electronic form on a VADOC workstation! State law prohibits all confined offenders from having access to the internet. The electronic form is web-based, and VADOC computers that are networked do have access to the internet!
The electronic form has tooltips for each field that describe what data should be recorded in each field. To view the tooltip, place the mouse’s cursor over the field. The tooltip will appear in a small, yellow pop-up box near the cursor.
Please PRINT A COPY of the report form for your records before you submit it. The electronic form prompts you to confirm that you have done so before letting you submit it. You’ll need to remember to run a copy if you’re faxing or mailing the hard copy form.
The form can support 4 “other” religion categories in the data table. If you have data for more religions than can be recorded on a single form (which is a total of 10), you should submit a second from with only the additional data. Please do NOT submit duplicate data.
The date on the electronic form defaults to the current date. (This can be overwritten, though, if you so desire.)
The Region field is updated automatically when a location/facility is selected.
